Athletes are often leaders on and off the field. Please provide one example of how you have demonstrated leadership in both areas.

Please explain how you pursue "healthy athletic performance" as a student-athlete.

In the off-season aside from high school season, I get into the gym and workout at least 3 days a week, not including tennis itself. I lift weights and work on my endurance with the bike, treadmill, and eliptical. Aside from those workouts, I practice my strokes indoors and outdoors 5-7 days a week.
