Additional Questions:
Why do you want to be recruited to play sports in college?

I love sports. Without them I feel empty and bored. I always try to be involved in something and I typically use my god given talent of athleticism to do so.

Athletes are often leaders on and off the field. Please provide one example of how you have demonstrated leadership in both areas.

Our linebacker had lost his cool in a game. I calmed him down and he went on to make the game winning sack. I also tutor him in school.

Please explain how you pursue "healthy athletic performance" as a student-athlete.

Every night after practice I go home to study and make sure I get enough sleep. I also use my free periods in school to study and do homework.

Please describe how you are involved in service in your community and why these efforts are important to you.

Every year in my high school we are required to complete at least 24 hours of community service.
