Additional Questions:
Why do you want to be recruited to play sports in college?

I would like to be recruited to play sports in college because I want to continue to play soccer in college and want to have the experience of being on a team.

Athletes are often leaders on and off the field. Please provide one example of how you have demonstrated leadership in both areas.

Please explain how you pursue "healthy athletic performance" as a student-athlete.

I value hard work both in the classroom and on the soccer field. However, I always like to keep a balance between work and training and playing. I also try to balance my work so that I can rest and go to bed at a reasonable time.

Please describe how you are involved in service in your community and why these efforts are important to you.

At school, I am a member of the Community Service Club. And in my community at home, I volunteer with Habitat for Humanity and am also involved with the Wheelchair Sports Federation.
