Why do you want to be recruited to play sports in college?

My goal is to play college lacrosse while becoming a physical therapist. I’ve lettered in football, hockey and lacrosse throughout high school, but I’ve chosen to focus on lacrosse because it’s what I was meant to do.

Athletes are often leaders on and off the field. Please provide one example of how you have demonstrated leadership in both areas.

I was team captain of my lacrosse team and I help train other employees at work.

Please explain how you pursue "healthy athletic performance" as a student-athlete.

I practice daily. I go to the gym when possible and I never use alcohol or drugs.

Please describe how you are involved in service in your community and why these efforts are important to you.

I work full time because I feel it’s my job to have a sense of responsibility before graduating high school.
